You don not have to include all dates needed just a range of dates i.e. (4/23/2019 - 11/30/2019) please include any special insurance requirements in the notes section

The Cost for Insurance for all youth clubs that are not affiliated with an Adult club or doesnt have a Divisional Minor Board must pay $250.00 each year.

If a club decides to acquire their own insurance, they will need to provide a copy of the insurance certificate to the USGAA Insurance officer, and American Gaelic Games will need to be named as additional insured. The insurance must be equal or greater than the USGAA's insurance.
